Oak Tree Supported Living provides specialist supported living and residential services for adults with learning disabilities, autism, and acquired brain injuries across Bristol and the Midlands.

About the Role

We are recruiting a Key Worker to join our Birmingham supported living service, holding a caseload of individuals with learning disabilities and/or autism. As Key Worker, you will be the named lead for a small group of people, taking responsibility for their care plan reviews, family communication, and day-to-day advocacy.

This is a mid-level role suited to someone who has experience in a learning disabilities setting and is ready to take on more responsibility.

What You Will Be Doing

  • Acting as the named Key Worker for a caseload of three to five supported individuals
  • Writing and reviewing person-centred support plans in collaboration with the individual, their family, and the wider MDT
  • Leading quarterly reviews and ensuring actions are implemented
  • Maintaining strong, open communication with family members and advocates
  • Supporting individuals in daily activities using active support principles
  • Mentoring junior support workers and sharing good practice
  • Attending multi-disciplinary team meetings and liaising with health professionals

What We Are Looking For

  • At least two years’ experience in a learning disabilities or autism support role
  • Understanding of person-centred planning and active support
  • Confident written communication — you can write clear, person-focused support plans
  • NVQ Level 3 in Health and Social Care (or working towards it)
  • Enhanced DBS on the Update Service
  • Full UK driving licence
